• image.dbo.hhq1_revised - all the bubble-derived data
• image.dbo.hhq1_revised_hospitalizations - hospitalization data and details

• Page One:
  • Recent hospitalization, if there is any

• Page Two:
  • Recent hospitalization, if there is any
  • Recent cancer treatment

• Page Three:
  • Recent heart diseases, if there is any
  • Recent cancer incidence, if there is any
  • Whether diagnosed with diabetes, stroke, or fracture of either the hip or the wrist
  • Screening for prostate and breast cancers

• Page Four:
  • Birth place
